Innovation award winner - Care Homes Medicines Optimisation - Eastbourne, Hailsham & Seaford and Hastings & Rother CCGs/ Sussex Community NHS Foundation Trust

On the 10th February, 1-2pm, we will be welcoming Kayt Blythin, from Eastbourne, Hailsham & Seaford and Hastings & Rother CCGs/Sussex Community NHS Foundation Trust to further capture and share their winning Innovation Award in October 2019, under the Care Homes Medicines Optimisation category - Self-care toolkit for Care Homes - Winner.

Kayt will present and share their experience of implementing a Self-care toolkit for care homes across a large geographical area working with over 200 homes and 42 GP surgeries.

Kayt will talk about the success stories as well as the lessons learned.

The presentation will be followed by a Q & A session.
