Advancing Reablement Services: A Flexible Approach to Medication Support and Independence (2025)

Project summary

Background:

Reablement services are designed to support patients in regaining independence and reducing care needs whilst remaining in their own home following significant health setbacks. 

A collaborative initiative between North of England Care Support (NECS) Medicine Optimisation Social Care Team and Northumberland STSS to review and update guidance for reablement services and promote flexibility in medication support thus ensuring a more person-centred approach to care.

Aim:

To review and update outdated guidance which limited the flexibility required to tailor reablement support to individual patients, whilst managing risk therefore ensuring more adaptable and person-centred support.
Methods:
Previous guidelines primarily focused on medication administration and reminders, limiting opportunities for broader rehabilitation efforts.

  1. Link with neighbouring Local Authority Reablement teams to establish current ways of working in the sector
  2. Update procedure to introduce new support activities which can be more responsive to individual requirements of patients, whilst still being compliant with NICE NG67
  3. Review 'Needs' assessment to ensure all necessary areas of medicines management are included
  4. Develop and deliver a training plan to inform staff team of new ways of working and allow opportunity for questions and discussions 

Intervention:

  1. Medication procedure reviewed and updated 
  2. Risk Assessment / Support Plan developed to evidence reasons for support needs in the various areas of medicines management.
  3. Explore staff hesitancy to broaden innovative care support solutions.
  4. Bespoke training program developed and delivered by the NECS Medicine Optimisation specialist team to staff, aiming to shift the prevailing risk-averse culture. The training sessions provided opportunity for staff to engage in open discussions, ask questions, voice concerns and seek solutions, fostering a culture of confidence and proactive support.
