BOB ICS Pharmacist-Led Integrated Severe Asthma Care Project (2024)

Project Summary

The Integrated Severe Asthma Care (ISAC) project aimed to address disparities in severe asthma management across the BOB Integrated Care System by improving access to advanced biologic therapies and FeNO testing.

Conducted from November 2021 to March 2024, the project deployed trained pharmacists to conduct outreach clinics in 47 general practices, which predominantly served diverse and socioeconomically disadvantaged populations.

These pharmacists used sophisticated patient prioritisation tools to identify individuals with potentially severe asthma, providing tailored care locally and facilitating expedited referrals to specialist consultants when necessary.

Over the course of the project, 141 patients commenced biologic treatments, and additional 91 had their pharmacotherapy successfully optimised within primary care settings. The ISAC garnered overwhelmingly positive feedback from patients, with 98% rating the service as 'very good', and primary care clinicians.

The project not only achieved significant health benefits by enhancing asthma management but also demonstrated a sustainable model that could be replicated in other healthcare systems.
