The care home team at Northamptonshire CCG have spent many hours setting up care home staff for online proxy ordering over the past 3 years. We help with the set up and the paper work and then pass it over to the surgery to continue.
It has become apparent over the pandemic that the homes we have helped set up over the years have not been maintained at practice level, resulting in some residents ordering via proxy, but the newer ones having only their individual logins, staff members have left and the account has not been deleted and no new staff members have been set up for proxy access in their place. There was no direct person in charge of this process in the practice and it was not being used to its full potential.
This gave us the idea of a care home champion at the surgery, who would be a first point of contact, continuity between the home and the surgery and also someone to maintain the proxy access online ordering. We thought this would be an opportunity to create improved communication between the home and the GP, plus improved care of the resident, by adding in transfer of care services conducted by the PCN pharmacist. It is was also the perfect opportunity to add in the synchronising of medicines to the cycle of the home, plus making sure the residential code for the resident has been added to their registration. We created a role that incorporated tasks that were not being maintained, whilst adding in tasks that were needed on a transfer of care.
