Project Summary

Cheshire Medicines Optimisation Teams implemented a project to improve cardiovascular outcomes by addressing multiple cardiovascular risk factors using a population health approach.

The project utilised technology to support medicines optimisation by developing a suite of risk-stratified searches to identify patients for review, an EMIS template to guide assessment of a range of cardiovascular risks and a software platform for monitoring and evaluation of the project.

The project aimed to implement the NICE-endorsed clinical pathway for lipid management for primary and secondary prevention of cardiovascular disease, and support practices to address other modifiable cardiovascular risks during the process. Overall, 130,000 clinical reviews were completed and Cheshire has increased the percentage of statins prescribed as higher intensity statins by 8%, improving the position from the 40th percentile in April 2022 to the 81st percentile in March 2023.
