Dietician role in medicines optimisation in care homes (2019)

Project Summary

A Dietitian was recruited on secondment for one year with the CCG. Part of the business case put forward was to ensure that residents are reviewed on an individual basis; because they are discharged from hospitals with lots of supplements which are not necessarily preferred formulary choice in primary care. Residents are not then reviewed or provided with dietetic follow up. Hence the project was set up to help tackle and improve these issues whilst working with the GPs and medicines optimisation in care home team.

Between February 2019 and May 2019, 24 training sessions have been delivered to care home staff across Mid Notts. This is in residential homes and nursing homes for older people. 13 sessions have been delivered in Mansfield & Ashfield CCG (89 staff consisting of carers, seniors, nurses, management and catering staff) and in Newark & Sherwood CCG 11 sessions have been delivered (45 staff consisting of carers, seniors, nurses, and management and catering staff).

Feedback received have been very positive, with many of the care home staff stating that they feel confident in measuring BMIs and monitoring MUST scores of residents and they know when to refer.
