The aim was to address the gap between patients identified with AF and those anticoagulated. Identification of those at high risk of stroke, anticoagulation in line with national guidelines, and appropriate clinical review and correct maintenance of anticoagulation.
The project was a combination of patient identification, audit, risk assessment, and clinical review by cardiologists for initiation or clinical pharmacists or GPs for dose correction. This was a large scale project taking over a year to complete across the CCG.
The outcomes were a significant shift in the management of AF and appropriate anticoagulation, addressing safety concerns on the correct dosing of anticoagulation and ultimately preventing 36 AF related strokes across the CCG.
