This project's aim was to tackle problematic polypharmacy by delivering risk stratified SMRs and to increase our team's confidence and competency at reviewing complex medicine regimes and deprescribing where appropriate.
In the process we planned to meet the PCN DES and SLA targets, to overlap with opiate work, high risk drug monitoring and ONS audit projects.
Overall, we aimed and achieved to reduce avoidable harm from non-beneficial medications, evidenced as shown in the power point presentation attached as PDF document.
