Project Summary

Quest for Quality medication reviews are provided for care home residents in Calderdale. The level 3 medication reviews are prepared by a pharmacist and consist of a multidisciplinary team meeting involving the pharmacist, a consultant geriatrician and the carers. The resident and their relatives are welcome to attend the review and sometimes a community matron or consultant psychiatrist will also be involved.

Access to primary and secondary care information and patient results allows full investigation into why a patient has been prescribed their current medication ensuring a more thorough review. The aim of the review is to optimise medication, while considering patient safety and deprescribing where appropriate.

Care home staff, who have been involved with medication reviews, will now contact the pharmacist with queries and concerns they have around a resident's medication which improves resident care. In the last financial year 93% of recommendations were accepted by the local GPs, leading to a saving in excess of £84k in prescribing costs.
