The introduction of a STOMP service into primary care (2019)

Project Summary

This project aimed to identify at-risk patients within DDES and North Durham CCGs using a data collection tool, and use the data gathered to inform the CCGs strategy to managing this patient group.

The data informed discussions within practices and with the local mental health Trust regarding patient safety and reducing risk to this group of patients from their medication.

This has led to the development and introduction of a newly commissioned specialist-led service to work within primary care, with current funding for 1 year, partly funded by Transformation Funding.

This service will be led by an independent prescribing Positive Behaviour Support (PBS) Nurse, in addition to a PBS band 5 nurse and will involve monitoring, review and where appropriate reduction in psychotropic medication in this patient group. It will also involve education of patients, staff and carers regarding the side effects of the medication, and appropriate treatment and management of behaviour that challenges.
