Using MedOptimise to record medicines optimisation in care homes (2019)

Project Summary

The project involved trialing an online tool in the CCG to record interventions from medication reviews in care homes thus making it easier to record, analyse and report data.

The MedOptimise Intervention Tool provides an all in one resource for Medicines Optimisation teams. It provides the following:

  • Quality, Risk and Savings outcomes recorded
  • Drug and Administrative Interventions recorded
  • Reliable and evidence based Hospital admissions prevented
  • Peer Review and Team Collaboration
  • Automative reporting
  • ACBS scores automatically calculated
  • Maps and contact details of all Practices and Care Homes
  • Complete Prescribing analysis tool
  • QIPP Savings identified
  • Risk scoring tool

It has been used to capture the reduction in polypharmacy in care homes, recording the number of medications stopped, started, dose reduction, switches and anticholinergic burden score reduction. It has also incorporated the STOPP/START and PINCER criteria.

The use of MedOptimise Interventions Tool has not only improved the accuracy and efficiency of the recording of interventions, but has enabled the recording of quality outcomes. This has moved the focus away from drug savings to admission avoidance and improved outcomes for the patient. As a result it has allowed the medication reviews to be more patient focused, holistic and ensuring the interventions are evidenced base.

It has significantly reduced the time spent recording, analysing and reporting the cost savings made from the interventions. Previously this was all done manually using the drug tariff in a self-made excel spreadsheet. The interventions can be easily filtered down into care home, GP practice, type of intervention and many more
This tool is now being rolled out across the Frimley Health ICS in order to standardise the way interventions from medication reviews in care homes are recorded and reported.
