The project has developed large-scale implementation plans to deliver bespoke message handling technology to safely and quickly transfer the discharge information from hospital to the patient's nominated community pharmacy.
Principal aim is to improve patient safety and free up resources by reducing re-admission of patients due to medication errors or adverse drug reaction (ADR) following discharge from hospital.
