On the 23rd January 1-2pm, we welcomed The Vanguard Care Home Pharmacy Team from NHS East and North Herts CCG to further capture and share their Innovation Award from October 2018 for Addressing Problematic Polypharmacy – Improving Health and Minimising Risk to Care Home patients.
Puja Vyas and Jodie White presented and shared their experience of Medicines Optimisation in Care Homes, Deprescribing and Reducing Waste. Eleesha Pentiah also presented and talked about the success stories as well as the lessons learned.
The presentation was followed by a Q & A session.
