Polypharmacy and deprescribing I considers national and international resources that help support the understanding of polypharmacy and safe deprescribing, the evidence base for deprescribing and the impact of frailty and multimorbidity on polypharmacy.
Polypharmacy and deprescribing II provides a practical guide to support deprescribing; it covers agreeing treatment goals, shared decision making, patient decision aids and tools to support medication review.
